If you use a Mistral AI model that is classified as a general-purpose AI model with systemic risk and have experienced a serious incident within the meaning of article 49 (b) of the AI Act, please use this form to report it to us.

Note: information included in this report may be passed on to the AI Office, and, as appropriate, to national competent authorities, as per our reporting obligations under article 55 (c) of the AI Act.
